The Care Coordinator will recommend and assist access to medical, educational, social, vocational, housing, and more to support program goals. This role involves initiating referrals to next level care, coordinating with physical health providers for whole-person care, and advocating in the best interest of clients.

Requirements

• Bachelor's degree with four years of experience in social services.
• Valid CA driver’s license and current auto insurance per contract requirements required.
• Cultural competency understanding required.

Nice To Haves

• Three years of experience providing case management to individuals with mental illness and/or substance use disorders preferred.
• Experience providing mental health and/or substance use disorder services preferred.
• Bilingual preferred.
• Knowledge of Microsoft Office and EHR software preferred.

Responsibilities

• Provide case management with crisis intervention, discharge plan, group & individual work, mental health/substance use disorder counseling, screening, and assist clients in meeting basic needs and symptom management.
• Complete accurate and timely documentation, meet minimum billable requirements for direct client services if contractually required.
• Facilitate placement including pre-placement visits, and transportation as needed.
• Shift coverage including part of the on-call team.
• Provide field-based services as required by program and client need.
• Maintain confidentiality of client’s care.
• Participate in scheduled program meetings.
• Other relevant duties as assigned.
