About The Position

We are seeking a full-time care coordinator to serve as part of our team on the Flourish Mobile Health Clinic. This person will provide trauma-informed, harm reduction-oriented care coordination services to vulnerable populations, including individuals affected by substance use disorders (SUD), homelessness, and limited access to healthcare. This position supports the clinic's interdisciplinary care model by providing care coordination specific to healthcare referrals and healthcare access, including but not limited to coordinating behavioral, primary care, and specialty health referrals. In addition, this person will provide crisis intervention, connect patients with community resources, engage in community outreach and coordinate on-site experiences for social work and public health students.

Requirements

Ability to convey information clearly and concisely to groups or individuals either verbally or in writing to ensure that they understand the information and the message. Ability to listen and respond appropriately to others. Ability to impact and influence others to further the institution’s strategic objectives.
  • Technical, Computer, and Software Skills: Ability to use Microsoft Office (Excel, PowerPoint, Word) and basic databases and software.
  • Experience working in a fast-paced environment and the ability to respond to high-pressure situations.
  • Ability to thrive in an environment of hard work.
  • Ability to work effectively with a wide range of constituencies in a diverse community.
  • Vocational or technical training or degree in Social Work or related area is required.
  • 1-3 years experience working with mental health, addiction, medical, and/or vulnerable populations is required.
  • The selected candidate will be required to complete a background check satisfactory to the University.

Nice To Haves

  • Prior experience with Salesforce would be considered a plus.
  • Bachelor's degree in Social Work is preferred.
  • 4-6 years experience is preferred.

Responsibilities

  • Direct patient care & support by assisting with providing screenings, brief interventions, and harm reduction to patients, in addition to maintaining accurate and confidential patient records and documentation.
  • Care coordination through development of care plans, coordinating referrals and follow-up, and assisting with required application submissions for community resources to meet individual needs.
  • Engage and build relationships with community partners to decrease barriers to care for patients.
  • Coordinate on-site experiences for social work, public health or other undergraduate students.
  • Respond to emergency situations which includes mental health, such as crisis intervention.
