About The Position

Under the direct supervision of the Crisis Center Nurse Manager, the Care Coordinator is responsible for assessing, planning, coordinating, monitoring, and linking clients with mental illness in the Crisis Center (involuntary extended observation unit and voluntary crisis residential unit) to service providers in the community after discharge. In addition, the position is responsible for developing the recovery plan and providing rehab services including individual and group skills training, supported employment, supported housing, crisis management and intervention services to the clients in the Crisis Center. Documentation and assessments must meet all applicable standards and established performance benchmarks. The position provides care to individuals with a mental illness and/or a co-occurring substance use/abuse diagnosis along with individuals with a dual diagnosis of mental illness and intellectual or developmental disability. Must be able to work within a multi-disciplinary treatment team. Must have excellent engagement, time management and organizational skills. PRN staff work 12-hour shifts on Saturday and Sundays, 8-hour day or eve shifts during the week to fill in as needed. Requires flexibility to meet program needs. PRN staff are required to work a minimum of one weekend shift a month and cover one holiday a year. The Care Coordinator reports directly to the Crisis Center Nurse Manager and is responsible and accountable for the adherence to and promotion of the compliance policy.

Requirements

  • Bachelor’s or advanced degree from an accredited college or university in Counseling, educational psychology, early childhood intervention, gerontology, human growth and development, medicine, nursing, physician assistant, Psychology, rehabilitation, social work, sociology and special education.
  • Texas driver’s license in good standing.
  • Minimum of 1 year case management experience.

Nice To Haves

  • 2 years’ experience in mental health crisis services
  • Spanish speaking
