About The Position

The Crisis Care Coordinator is responsible for providing follow up services for individuals seen in crisis and helping coordinate referrals and warm hand offs to appropriate services in the community to help stabilize and/or mitigate future crisis. The incumbent provides direct assistance in gaining access to services; coordinates care within and across systems; ensures linkage to appropriate services; conducts assessment of the eligible recipient to determine service needs; evaluates and completes a risk assessment; collaborates with law enforcement and other first responders to educate; works closely with all member of the crisis team and co-responders to help divert individuals from incarceration or unnecessary visits to the emergency department.

Requirements

  • Bachelor’s degree in social sciences, mental health or related field required
  • Washington State Counselor Agency Affiliated Registered (ACCR) credential must be applied for within thirty (30) days of hire. Employees may work while the application is pending for up to one hundred twenty (120) days, OR a Mental Health Counselor Assoc (LMHCA) credentialed with Washington State may be substituted for the AAC
  • Current Washington state driver’s license
  • Proof of a safe driving record (which meets established MultiCare Standards) obtained by a motor vehicle report from the appropriate state
  • Must be approved, have clearance, and pass additional background checks from correctional facilities and jails that make referrals to crisis team and agree to annual background checks

Nice To Haves

  • Master’s degree preferred
  • Minimum two (2) years of experience preferred
