About The Position

Inova Health Medical House Calls is seeking a full-time, day-shift Care Coordinator. The Patient Care Coordinator III provides clinical coordination and facilitation services for patients entering the healthcare system. The role works in a dynamic, team-focused environment and requires strong organization and effective communication in person and through written and verbal electronic channels. It also requires proficiency in call center operations, medical knowledge, and hospital policies and procedures.

Requirements

• At least 2 years of experience in a call center, customer service, or related profession.
• High school diploma or equivalent.
• Proficiency in call center operations, medical knowledge, and hospital policies and procedures.
• Strong organizational skills and effective in-person and written and verbal electronic communication skills.

Responsibilities

• Answer and mitigate calls and provide intake services on specialty lines.
• Interpret patient conditions, complaints, and diagnoses to route patients to the appropriate subspecialty service at client facilities.
• Coordinate with physicians, nurses, and other staff to facilitate smooth admission processes.
• Read, review, and interpret documents requesting services.
• Interact with client facility staff by telephone to intake calls.
• Maintain multiple databases relevant to the service line.
• Accurately enter calls for service into a Computer Aided Dispatch (CAD) system when required.
• Search for and enter patient information accurately into client Electronic Health Records (EHR).
• Document and report shift activities using spreadsheets, worksheets, and email formats.
• Notify management in real time of problems, concerns, and compliments received.
• Generate performance reports using computer software.
• Perform additional duties as assigned.
