About The Position

Terros Health is seeking a Care Coordinator for the Transitional Age Youth (TAY) program, specifically within the Families FIRST initiative in Phoenix, AZ. This role is crucial in assisting families involved with the Department of Child Safety (DCS) to overcome substance abuse barriers, achieve family reunification, and attain self-sufficiency. The Care Coordinator will collaborate closely with DCS, clinical teams, and other providers to coordinate services and support families in meeting their case plan goals. Responsibilities include conducting monthly meetings to track goal progress, providing resources and skills training to promote home stability, and engaging patients/residents in appropriate services. The ideal candidate will be professional, friendly, organized, compassionate, and a self-starter. This position reports to the AFF Care Coordinator Manager.

Requirements

  • An associate’s degree in human services or behavioral health field, and at least three years of related experience including working with multi-problem families in the field of substance abuse, child welfare or behavioral health; OR a bachelor’s degree or higher in a behavioral health field.
  • Knowledge in use of electronic health record (NexGen a plus).
  • Knowledge of word processing systems, i.e., Microsoft Word, Access, and Excel.
  • Ability to type 55 wpm.
  • Ability to read and compose documents in a professional and technically correct manner.
  • Good communication skills and ability to work well with individuals and groups.
  • Demonstrate time management, organization, conflict resolution and judgment skills.
  • Working knowledge of the child welfare/child safety system.
  • Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health’s driving policy.
  • Must be able to pass the Arizona Department of Child Safety Direct Service Central Registry Clearance without any disqualifying events.
  • Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
  • Must pass a TB Test.

Responsibilities

  • Responsible for initial outreach for all minors and TAY patients referred to AFF, including obtaining necessary paperwork and scheduling assessments.
  • Responsible for ongoing care coordination for TAY clients, including completing a Short-Term Service Plan within three days of assessment.
  • Conduct meetings or conference calls with the Service Team (patient, family, DCS worker, other agencies) to discuss and finalize the Service Plan within 15 days of the comprehensive assessment.
  • Schedule and conduct monthly Service Plan reviews with the Service Team to monitor progress, identify completed goals, and update the Service Plan quarterly.
  • Upload required documents into Guardian within prescribed time frames.
  • Monitor the random alcohol and drug testing plan for assigned patients, coordinating with the Extended Foster Care Coach and/or DCS specialist on missed or positive tests.
  • Provide concrete supportive services based on the needs of the young adult to achieve sobriety (e.g., car repair, housing assistance, utility assistance, clothing, food, phone).
  • Conduct outreach and re-engagement activities as described in the AFF Program Manual prior to closing a case.
  • Complete required documentation in patient records, including Progress Notes, meeting production metrics.
  • Drive to various locations throughout the assigned territory to visit and provide services to patients.
