About The Position

Community Counseling Solutions provides a team-based Servant Leadership environment! This position provides individual, family and group social work services and other social work treatment interventions to clients, including social work assessment, assisting with civil commitment and hold process, and discharge planning.

Requirements

  • Bachelor’s degree in psychology, social work, behavioral science or related field or bachelor’s degree in nursing and licensed by the state; or have a combination of three years experience.
  • Possess or have the ability to obtain MHACBO QMHA certification within 30 days of hire.
  • Education and experience which demonstrates the competencies to identify precipitating events, gather mental and physical history including drug and/or alcohol use, past mental health services and criminal justice contacts; assess family, social and work relationships, conduct a mental status examination, complete assessment documentation, and write and supervise a treatment plan.
  • Certification from the state of Oregon if degree is in nursing.
  • Knowledge of normal and abnormal human development and behavior.
  • Knowledge of recognized treatment interventions.
  • Knowledge of casework techniques and program standards.
  • Knowledge of laws and regulations as they apply to the clinical setting.
  • Knowledge of community resources, eligibility requirements, and procedures to obtain needed services for clients.
  • Skill in developing and maintaining a therapeutic relationship with mentally ill patients.
  • Ability to act as advocate for client.
  • Ability to communicate effectively in both written and oral formats. Ability to present and exchange information internally across teams and co-workers, and externally with client family members and the public, at times under difficult or emotional circumstances. Information communicated ranges from routine/basic to complex and confidential information.
  • The position requires the handling of highly confidential information. Must adhere to rules and laws pertaining to client confidentiality.
  • Must have good typing skills, have knowledge of basic office software programs and ability to effectively use such software.
  • PRE HIRE DRUG SCREEN REQUIRED
  • Must hold a valid driver’s license as well as personal auto insurance for privately owned Vehicles utilized for CCS business such as client service purposes, travel between business offices and the community, to attend required meetings and trainings.
  • Must show proof of $300,000 or more liability coverage for bodily injury and $100,000 or more in property damage and maintain said level of coverage for the duration of employment at CCS.
  • The employee’s insurance is primary with CCS insurance being secondary. CCS reserves the right to deny any employee the use of a vehicle owned by CCS.
  • Must pass all criminal history check requirements as required by ORS 181.536-181.537 and in accordance with OAR 410-007-0200 through 410-007-0380.
  • In addition to a pre-employment background check, each employee, volunteer and contractor shall be checked on a monthly basis against the OIG and GSA exclusion lists, as well as other federal and state agency lists. If it is discovered that an employee, volunteer or contractor is excluded or sanctioned it will be the cause for immediate termination of employment, volunteering, or the termination of the contract.

Responsibilities

  • Performs social work assessments that are based on client interview and interviews with family members, friends or other contacts in the community.
  • Provides client’s family with information regarding client’s progress and treatment options.
  • Researches and compiles information regarding the services available to the client. Prepares applications for financial and program benefits and assists client with the management of financial resources as needed. Assists the client in obtaining social, medical and legal services.
  • Acts as patient advocate in obtaining needed services.
  • Participates as a member of the treatment team to determine eligibility of clients for admission. Identifies community resources available as an alternative to admission, including community home placement. Works with treatment team to formulate and periodically evaluate a plan of treatment.
  • Performs activities related to civil commitment and hold process, including processing appropriate paperwork and documentation, and coordinating with the courts and other community partners.
  • Monitors services delivered to client to ensure that the client’s needs are met.
  • Coordinates placement of clients into community programs and/or makes referrals to programs that provide other levels of care.
  • Develops and implements discharge/aftercare plans for all clients.
  • Assists client in planning return to community and arranges needed resources for continuing care and support.
  • Assembles all information and paperwork necessary for referral packets.
  • Maintains on-going documentation of social work intervention, planning and outcomes by preparing reports and making necessary entries to treatment plan documents.
  • Acts as community and family liaison by initiating and maintaining necessary contact with the client’s family, the courts, district attorneys, applicable county, state and federal agencies, residential care facilities, and related resources and regulatory agencies.
  • Maintains records and prepares reports and correspondence. Compiles data and conducts studies on program effectiveness via follow-up of discharged clients.
  • Participates in seclusion and restraint practices, as necessary.
  • Performs related work as required.
  • Other duties as assigned.
