Pathways to Success is a highly structured program implemented by HFS for individuals under the age of 21 who are Medicaid eligible and meet specific criteria. This program offers intensive case management and wraparound services to eligible clients and their families. The Care Coordinators link families to traditional outpatient services as well as Pathways-specific services. The Intensive Care Coordinator (CCSI) is primarily responsible for facilitating the care coordination process for children with mental health diagnoses and their families. This involves leading Child and Family Team Meetings, coordinating with professionals, and assisting the child in achieving their goals. CCSI services are provided to children stratified into Tier 2. Designated CCSI Care Coordinators manage an average caseload of 16 families, never exceeding 18 families at once. The CCSI empowers families to develop a positive future outlook and utilize a strength-based empowerment model to improve their child's functioning in the home, school, and community.
