About The Position

The Care Coordinator II supports care management activities and assigned teams to help ensure services are delivered by healthcare providers and partners and to promote continuity of care and member satisfaction. The role engages members through telephone outreach or home visits, documents care and service plans, and supports coordination of the services and resources members need. The position requires residence in Texas. Computer skills are required; a healthcare background is strongly preferred.

Requirements

High school diploma or GED.
1–2 years of related experience.
Must reside in Texas.
Computer savvy.

Responsibilities

Contact members by phone or through home visits to discuss care or service plans, next steps, resources, questions, and concerns, and provide ongoing education as appropriate.
Coordinate care activities according to care or service plans, working with healthcare and community providers, partners, members, and caregivers to accommodate changes or progress.
Respond to member and provider inquiries, requests, and concerns related to care or service plans.
Communicate with care managers, practitioners, and others to facilitate member services and continuity of care.
May assist with service assessments or screenings and document members’ care needs.
Document and maintain member records in accordance with state and regulatory requirements, and distribute records to providers as needed.
Follow standards of practice and policies that comply with contractual requirements and regulatory guidelines.
Identify member needs and make referrals to Care Managers, community-based organizations, and Disease Managers.
Educate members about available benefits and resources.
Perform other duties as assigned and comply with all policies and standards.
