As a Care Coordinator at the Regional Crisis Stabilization Center (RCSC), you will work closely with a multidisciplinary team to help clients access services, coordinate care, and achieve positive outcomes. Requirements

  • Master's Degree in Human Services.
  • One (1) year experience providing service collaboration and coordination, psychoeducation, and behavioral health skills trainings to the client population assigned.
  • Must be registered with the Board of Counseling to practice as a QMHP, or register as a QMHP - Trainee within 30 days of employment.
  • Must successfully pass a criminal history fingerprint background investigation, DMV record check, Child Registry search, drug screening test and employment reference checks.

Responsibilities

  • Coordinating referrals and transitions of care
  • Serving as a liaison between RCSC and community programs
  • Supporting discharge planning and aftercare coordination
  • Completing case management activities (including benefits applications) and accurate electronic record documentation
  • Collaborating with internal/external providers
  • Reviewing pre-screening evaluations for appropriateness
  • Developing client service plans
  • Providing direct crisis intervention support
  • Maintaining confidentiality and advocacy in accordance with policy
