Care Coordinator - Crisis Stabilization

Requirements
- Master's Degree in Human Services.
- One (1) year experience providing service collaboration and coordination, psychoeducation, and behavioral health skills trainings to the client population assigned.
- Must be registered with the Board of Counseling to practice as a QMHP, or register as a QMHP - Trainee within 30 days of employment.
- Must successfully pass a criminal history fingerprint background investigation, DMV record check, Child Registry search, drug screening test and employment reference checks.
Responsibilities
- Coordinating referrals and transitions of care
- Serving as a liaison between RCSC and community programs
- Supporting discharge planning and aftercare coordination
- Completing case management activities (including benefits applications) and accurate electronic record documentation
- Collaborating with internal/external providers
- Reviewing pre-screening evaluations for appropriateness
- Developing client service plans
- Providing direct crisis intervention support
- Maintaining confidentiality and advocacy in accordance with policy
