Southern Illinois University Edwardsville (SIUE) is seeking a Care Coordinator for Counseling Services. This role is crucial in assisting students with navigating behavioral health care and ensuring they remain connected to necessary support systems. Operating within a stepped-care and walk-in access model, the Care Coordinator will help students identify appropriate care levels, overcome treatment barriers, and connect with campus and community resources. The position involves providing behavioral health screening, triage, case management, care coordination, crisis support, and resource navigation. The Care Coordinator will manage a caseload, offer ongoing follow-up and continuity of care for students with complex needs, and assist in overcoming barriers such as insurance, transportation, financial, scheduling, and access issues. This role also includes coordinating warm handoffs and referrals to various services, supporting students transitioning from higher levels of care, serving on the university CARE Team, conducting mandated assessments, and collaborating with partners to develop support plans. Building and maintaining relationships with community providers and facilitating psychoeducational workshops are also key responsibilities. The position functions under clinical supervision and does not provide ongoing psychotherapy.
