The Care Coordinator II performs a clinical role that is vital in support of the work of RN coordinators and licensed independent practitioners (LIPs) through independently performing a variety of clinical, administrative, patient experience and technical functions. Care coordination is the task of bridging gaps between facilities, specialists, labs, community resources, and primary and dental care. The Care Coordinator II works in the Harold Schnitzer Diabetes Health Center/Adult Endocrinology Clinic and primarily supports the adult and pediatric diabetes clinical practice, supporting the work of the licensed independent practitioners, RN & RD diabetes educators. Successful coordination of complex DME and medication authorizations and fulfillment is critical, as well as coordination of remote patient data portal management. This individual will also coordinate specialized Endocrinology genetic testing and inpatient admissions. Lead on handling urgent out of insulin refills and critical lab result follow up.
