About The Position

Excelsior Wellness Center is more than just a place for treatment; we are a community dedicated to helping individuals and families find their path to wellness. We believe in the potential of every child and family to live safer, stronger, and more fulfilled lives. The Care Coordinator works with a multi-disciplinary team including a mental health therapist, a youth or family peer specialist, therapeutic recreation specialists, and education and healthcare professionals to engage and support families. This position provides a collaborative forum to engage a variety of child-serving partners and advocate for the needs of individuals and their families to support the family remaining together and to improve the safety and satisfaction in their lives. This role provides direction in the development and implementation of cross-system care plans that are designed to help the youth and family avoid out-of-home placement, end multi-system involvement, and incorporate additional resources and supports as necessary.

Requirements

  • High School Diploma required
  • Registered Agency Affiliated Counselor Credential or ability to become registered within 60 days required.
  • Comprehensive knowledge of community resources and public systems.
  • Exceptional communication and interpersonal skills.
  • Must have strong multitasking and organizational skills.
  • Ability to work in a fast-paced environment and make ethical and safe decisions.
  • Ability to manage individual stress and perform essential job functions in a safe and professional manner.
  • Must be able to pass a criminal background check.
  • Must be 21 years or older.
  • Valid state driver’s license.

Nice To Haves

  • BA in counseling or related field preferred.
  • Minimum three years of working with youth and families preferred.
  • Community Health Worker certification preferred.

Responsibilities

  • Adheres to WISe program timelines for completion of care coordination tasks.
  • Participates in the on-call rotation to ensure families have access to crisis services 24/7. This may include phone support and in-person response.
  • Provides direction in the development and implementation of community resource programs to achieve desired outcomes for children and families.
  • Understands and actively promotes WISe programs throughout the county, identifying ways in which efforts aimed at improving outcomes for children and families can be coordinated.
  • Recognizes barriers that occur in a system of care and develop and implement practices necessary to sustain system of care improvements.
  • Assesses opportunities and barriers to achieve WISe services across all systems.
  • Develops long and short-term goals and objectives for the development and implementation of agencies WISe program services across all systems of care.
  • Seeks opportunities to effectively communicate with the public and private child and family serving agencies to share information about the program.
  • Serves as liaison between all child and family serving agencies and the WISe treatment team.
  • Coordinates cross-agency communication and planning as needed.
  • Uses company vehicles when transporting clients/families to program activities.
  • Complete all training and demonstrates competency in skills necessary for role.
  • Is aware of and available to all WISe services as needed for additional support in periods of crisis.
  • Maintains professionalism and unity when communicating with clients, their families, and other representatives; preserves client confidentiality at all times .
  • Attends and actively participates as a representative of Excelsior and its subsidiaries in meetings, both internal and external.
  • Effectively represents Excelsior in the community, to include but not limited to the City of Spokane, the State of Washington and other venues as attended.
