About The Position

360 Behavioral Health is seeking a Care Coordinator to join their clinician-led team in Fresno, CA. This part-time role involves overseeing Direct Support Professionals (DSPs), collaborating with families and Regional Centers, and ensuring clients receive high-quality, compassionate care. Requirements

• 1+ year clinical experience working with individuals with diverse needs
• Strong leadership, organization, and communication skills
• Ability to work independently and collaborate with a multidisciplinary team
• Comfortable in diverse home environments and culturally responsive
• Bilingual Spanish Required
• BA/BS required
• Must pass background check/Live Scan
• TB clearance, CPR/First Aid, and Crisis Prevention Certification

Responsibilities

• Supervise and coach DSPs to deliver exceptional services
• Ensure families receive consistent, high-quality care
• Collaborate with Regional Centers, clients, and behavioral agencies
• Conduct staff meetings, professional development, and utilization reviews
• Provide clinical guidance and mentorship
• Maintain accurate documentation and communication
