This is a clinical role that is vital in supporting the work of the physicians, advanced practice providers, and nurse team within the Urology Department in partnership with the Oncology program. The Care Coordinator will work in close collaboration with providers, surgery schedulers, and care coordination team in a variety of clinical administrative and technical functions. The Care Coordinator I performs a clinical role that is vital in supporting the care team with coordination, analytical and administrative functions. Care coordination in panel management is the task of bridging gaps between patient care, specialists, labs, health maintenance and preventive care. This is the first of a two-level series. It is distinguished from the higher level by the need for greater supervision, less frequent direct patient interaction, and a narrow scope of duties and expectations.
