About The Position

This is a clinical role that is vital in supporting the work of the physicians, advanced practice providers, and nurse team within the Urology Department in partnership with the Oncology program. The Care Coordinator will work in close collaboration with providers, surgery schedulers, and care coordination team in a variety of clinical administrative and technical functions. The Care Coordinator I performs a clinical role that is vital in supporting the care team with coordination, analytical and administrative functions. Care coordination in panel management is the task of bridging gaps between patient care, specialists, labs, health maintenance and preventive care. This is the first of a two-level series. It is distinguished from the higher level by the need for greater supervision, less frequent direct patient interaction, and a narrow scope of duties and expectations.

Requirements

Three years’ experience in a hospital or hospital based clinic setting
High School diploma or equivalent
Positions with in person patient care: BLS certificate within 30 days of hire or prior to independent practice, whichever comes first
Completion of a nationally recognized accredited medical assistant training program, including a practicum (externship) of at least 160 hours
Successful completion of a formal medical services training program of the United States Armed Forces
Current Oregon Emergency Medical Technician (EMT) license (basic or advance) and national EMT registration with the National Registry of Emergency Medical Technicians (NREMT)
Current Oregon Practical Nurse License
Completion of a Dental Assistant training program and successful completion of the DANB exam
MA certification received from a nationally recognized and accredited certifying body, upon hire or by the completion of the probationary period or internal job change evaluation period, as appropriate.
Ability to work independently and as a member of the team, including leadership.
Knowledge of patient flow and back office functions.
Ability to effectively and efficiently perform chart scrubbing and panel outreach activities.
Demonstrated strong analytic skills, including displaying and interpreting data.
Proficiency with EPIC, Microsoft Office including Excel, Word and Power Point.
Ability to manage time sensitive competing demands and meet deadlines.
Working knowledge of medical terminology.
Strong relationship building skills, excellent communication skills, exceptional customer service skills, highly sensitive to patient needs.
Demonstrated knowledge and attention to the needs of Special Needs patients and their caregivers and families.

Responsibilities

Uses EPIC for report writing to reach out to patients by making cold calls, My Chart messages, sending letters.
Supports the team by entering referrals, orders and interacting with the care team to ensure health maintenance and preventive care is provided.
Scrubs (reviews and manages) charts daily to understand what is and is not needed by each scheduled patient.
Proactively schedules appointments.
Assumes a lead role in the work for health maintenance and panel coordination.
Assists with questions regarding referrals and tests.
Provides information and education to patients and gathers information for specific visits.
Prioritizes and responds to patient questions via MyChart, phone, email and fax in regards to scheduling and care issues within scope.
Escalates all issues outside scope as appropriate.
Documents legibly, concisely, correctly and completely to ensure team can accurately assess the status of care needs.
Uses approved clinical resources and follows procedures appropriately.
Participates as assigned with database input.
Issues patient safety alerts to providers and helps manage the patients affected.
Monitors department tracking tools and provides providers with real-time Function/Duties of Position Patient management - Responsible for coordinating and managing the non-nurse aspects of the patient's clinical care process.
Scrub schedule and prep charts for new patients - ensure records are ready and patient is properly prepared for clinic visits.
Reviews the plan of care to ensure it is being executed correctly.
Works with the provider by providing support with order entry, Medication refills and provides non nursing support and education to the patient and family.
Places lab and scheduling orders and follows up with patient to communicate appointment time.
Monitors the Epic inbasket for messages that need attention from the patient, providers, and nurse team and handles messages within certification scope.
Monitors Epic for refill requests and manages appropriately.
Perform authorizations and refill requests as appropriate, provides leadership, expert guidance and oversight of clinic flow.
Supports daily operations across front and back office.
Ensures that clinic flows smoothly and resolves flow issues proactively, escalating to Clinical Operations Supervisor, as needed.
Ensures patients follow through with outside referrals to other specialties, labs and diagnostic imaging.
Assumes a lead role in the work for health maintenance and panel coordination.
Assists with questions regarding referrals and tests.
Document legibly, concisely and completely to ensure that others can accurately assess the status of patients' progress by reviewing that documentation.
Participate as assigned with patient chemotherapy timelines.
Monitors the active department tracking tools and provides real-time updates.
Initiates and implements processes to facilitate new protocol implementation and tracking tools.
Attends regular meetings with providers and care team to develop and review plan of care with care team.
Orders and schedules ancillary services and procedures and communicates to the patient.
Coverage for nurse team as appropriate on day off to be in Outlook email, Epic Inbasket and phone throughout the day.
Other tasks and duties as assigned.
Miscellaneous department assistance such as helping out in other sections when needed to ensure overall successful clinical operations.
