About The Position

Join our team as a Care Coordinator and make a meaningful impact by connecting patients with the specialized eye care they need. In this fast-paced role, you'll work collaboratively with our Triage Team to coordinate referrals, schedule appointments, and deliver an exceptional patient experience through outstanding communication, organization, and customer service. The Care Coordinator works directly with the Triage Team to review and process incoming referrals to ensure timely and appropriate care for referred patients.

Requirements

  • High school graduate or high school equivalency credential.
  • Minimum 1 year experience performing medical office scheduling or related experience in a health care setting.
  • Strong computer and technical skills including but not limited to: electronic medical records systems, Microsoft Office and email.
  • Builds rapport face-to-face and over the phone in order to deliver excellent customer service skills.
  • Applies effective communication and interpersonal skills necessary for engaging professional courtesy with patients, physicians, supervisors, and other staff.
  • Maintains patient confidentiality per state and federal and practice regulations.
  • Performs responsibilities and make well-reasoned decisions, both independently and as part of a team.
  • Ability to work well under pressure and remain calm in highly stressful daily situations.
  • Projects and maintain a professional image.
  • Self-motivated and completes work in a timely, accurate, and thorough manner.

Responsibilities

  • Works with the Triage team to facilitate patient access to EMG’s services.
  • Ensures all registration information is received and entered into Nextech prior to the appointment including optometrist, PCP, specialist(s), insurance information and insurance referral information when required
  • Documents the “notes” section of the patient account regarding any pertinent information.
  • Documents all patient interactions within the patient’s medical record.
  • Schedules new patient appointments to the mutual satisfaction of the patient and provider.
  • Provides patient education regarding what to expect as part of their care at EMG, including preparation and mailing of new patient paperwork.
  • Communicates to patients in a manner that instills understanding and confidence about their upcoming appointment.
  • Communicates with referring Optometrists with their patient’s appointment information.
  • Provides a high level of customer service by demonstrating compassion and caring in dealing with others.
  • Scans paper medical records into the patient’s electronic medical record.
  • Monitors and manage medical information shared electronically (EMR, virtual faxes) in a timely manner.
  • Participate in duties interdepartmentally as deemed necessary, by a member of management and/or doctor(s).
  • Demonstrates flexibility to meet changing demands on daily basis while also accomplishing tasks.
  • Demonstrates the ability to re-prioritize workloads and assignments for the team on short notice.
  • Attends meetings, as assigned, and participates in educational activities to keep skills up to date.
  • Maintains open communication with PSRs and Surgical Planning teams to maximize provider schedules.
  • Performs other duties necessary to maintain the overall efficiency and continuity of EMG operations.
  • Monitors operating standards and report issues to leadership in a timely manner.
