About The Position

Lubbock Heart Hospital, owned by Surgery Partners, is seeking a Clinical Documentation Integrity (CDI) & Care Coordination Specialist to support patient-centered care, documentation excellence, and effective care transitions across the continuum of care. This onsite role combines expertise in clinical documentation improvement, care coordination, utilization management, and discharge planning. The ideal candidate will bring a blend of social work or case management experience along with CDI knowledge to enhance documentation accuracy, support regulatory compliance, optimize communication between providers and interdisciplinary teams, and improve patient outcomes.

Responsibilities

  • Review inpatient medical records concurrently to identify documentation opportunities that accurately reflect patient severity of illness, risk of mortality, and complexity of care.
  • Collaborate with physicians, advanced practice providers, nursing leadership, and case management teams to improve clinical documentation quality and completeness.
  • Initiate and manage compliant documentation clarification queries when appropriate.
  • Monitor documentation trends and identify areas for education and process improvement.
  • Support regulatory and accreditation requirements related to clinical documentation.
  • Assist in maintaining documentation practices that support accurate coding, reimbursement, quality reporting, and performance metrics.
