This is a temporary, full-time (40 hours/week) position with an estimated assignment length from July 2026 through December 2026, dependent on business needs. While the assignment is with the Alliance, the selected candidate will be an employee of a temporary employment agency. The role reports to the Medicare Care Management Manager (RN) and focuses on developing and managing individualized intervention plans for Medicare Dual Eligible Special Needs Plan (D-SNP) members. The goal is to promote optimal, achievable outcomes in a cost-effective and appropriate manner. The position involves working with an interdisciplinary Care Coordination team to facilitate quality outcomes across the continuum by educating members, caregivers, providers, external agencies, and internal teams, and assisting in navigating the managed care system. Participation in program evaluation and quality improvement efforts is also required to ensure D-SNP program effectiveness and regulatory compliance.
