This is a temporary, full-time (40 hours/week) assignment estimated to last from July 2026 through December 2026. The assignment length is dependent on business needs and dates may change. While the assignment would be at the Alliance, the selected candidate would be an employee of a temporary employment agency. The role focuses on developing and managing individualized intervention plans for Medicare Dual Eligible Special Needs Plan (D-SNP) members, facilitating quality outcomes through education and navigation of the managed care system, and participating in program evaluation and quality improvement efforts.
