About The Position

Baptist Health is seeking a full-time Care Coordination Assistant at its Baptist Metro campus in Jacksonville, Florida. Under the direction of a licensed professional, this role provides non-clinical and clerical support to the Pediatric Social Work and Care Coordination teams to help facilitate coordinated, patient-centered care. The position supports smooth care transitions for patients and families.

Requirements

• 1–2 years of hospital experience.
• High school diploma.
• Basic Life Support (BLS).
• Driver's license.

Responsibilities

• Provide non-clinical and clerical support to the Care Coordination and Pediatric Social Work teams.
• Assist with coordination of care and discharge planning activities.
• Support patient and family care transitions across the continuum of care.
• Communicate with internal departments and external agencies as directed.
• Maintain accurate documentation and assist with data entry in the electronic medical record.
• Coordinate meetings, referrals, and follow-up appointments as needed.
• Ensure timely completion of assigned administrative tasks.
