The primary role of the Behavioral Health Care Coordinator is to provide care management and administrative support to help patients access behavioral health, substance use, and social services tailored to their individual needs. This position supports the Behavioral Health (BH) team by facilitating service delivery, advocacy, and education for clients and the community. The Behavioral Health Coordinator also assists patients in connecting with additional resources, such as psychiatric services, referrals for a higher level of mental health care, and other community-based support services.
