The Baltimore City Health Department (BCHD) seeks a Grant Service Specialist II Cardiovascular Disease Program Coordinator to work with the Office of Chronic Disease Prevention in the Bureau of Health Promotion and Education. Founded in 1793, BCHD is the oldest health department in the country, with over 800 employees and an annual budget of $208 million, that aims to protect health, eliminate disparities, and ensure the well-being of every resident of Baltimore through education, advocacy, and direct service delivery. BCHD's wide-ranging responsibilities include maternal and child health, youth wellness, school health, senior services, animal control, restaurant inspections, violence prevention, emergency preparedness, STI/HIV treatment, and acute and chronic disease prevention. The Baltimore City Health Department is seeking a Grant Service Specialist II to act as the Cardiovascular Disease Program Coordinator in the Office of Chronic Disease Prevention. The GSS II will collaboratively develop a standardized process to identify individuals at risk for hypertension, screen them for social support needs (with a specific focus on food security) and provide navigational support and assistance. This GSS II position is a Community Health Worker (CHW) position and will connect participants currently enrolled in the Healthy Heart Ambassador Program and other lifestyle change programs to services that address social determinants of health, which may not be met in their current program. The GSS II will provide education, encouragement, and support to participants who engage in any of the above programming. The selected candidate will be supervised by the director of the Office of Chronic Disease Prevention.
