About The Position
The cardiothoracic nurse navigator serves as a consistent resource to support optimal care delivery for cardiac patients and other projects as assigned, focusing on coordination of care during pre-procedure, hospitalization, discharge planning and facilitating post-hospitalization follow-up. The nurse navigator is knowledgeable of multiple treatment decisions, cardiac evidence-based standards, best practice guidelines, and maintains care collaboratively across the continuum. Requirements
- Bachelor's Degree in Nursing.
- California Registered Nurse License upon hire.
- National Provider BLS - American Heart Association upon hire.
- 3 years Cardiovascular experience.
Nice To Haves
- Master's Degree in Nursing.
- Nurse navigator experience.
