The successful candidate will work with older adults and their families/caregivers living with or at risk of responsive behaviours associated with complex and challenging mental health and addictions issues, dementia or other associated neurological conditions. The BSO Clinical Transitions Facilitator will serve as the lead clinician for transitions focused on supporting high risk older adults requiring intensive transition support during long-term care home (LTCH) admission and across the system of care. The transitions clinician will be a member of the Seniors Mental Health and Addictions Response Team (also known as the BSO Mobile Team) and will work in collaboration with acute and community providers to ensure seamless care transitions between care sectors and providers. This program will service Huron and Perth Counties which includes supporting individuals, their families and/or caregivers at many different service access points and at times transporting patients and or accompanying them on transport. Travel throughout Huron and Perth counties is required for this position.
