The Department of Health and Human Services (DHHS) is seeking a Brain Injury Case Coordinator to be responsible for the coordination of supports for individuals seeking services. This role involves providing an understanding of the Brain Injury Waiver application process and eligibility criteria, assisting individuals with referrals, intake, and eligibility for the Brain Injury Waiver, and developing preliminary care plans with members/guardians using person-centered planning. The Case Coordinator will also work with Brain Injury providers to coordinate and negotiate required services to implement care plans. The workload includes helping applicants complete the Brain Injury Waiver Application, processing, logging, and documenting the status of these applications, and participating in teleconferences for members placed out of state while documenting their progress in treatment. This is a full-time position located in Augusta, Maine, with potential for partial telework upon management approval. The position requires some statewide travel.
