About The Position

This position is a full-time position located in Augusta. This opportunity allows partial telework with management approval. As a Brain Injury Case Coordinator, you will be responsible for the coordination of supports for individuals seeking services, while providing an understanding of the Brain Injury Waiver application process and eligibility criteria. You will also aide individuals with referrals, intake, and eligibility for the Brain Injury Waiver, develop preliminary care plans with member/guardians using person centered planning, and work with Brain Injury providers to coordinate and negotiate required services to implement care plans. Your workload will include helping applicants and assisting them in completing the Brain Injury Waiver Application, processing, logging, and documenting the status of Brain Injury Waiver Applications, and participating in teleconferences for members who are placed out of State while documenting their progress in treatment.

Requirements

  • Bachelor's Degree from an accredited educational institution in Social Work/Social Welfare; OR a Bachelor's Degree in a related social service/social welfare/social work area which includes at least 12 courses in behavioral science, social science, or social work
  • Must have or be eligible for conditional or full licensure as a Licensed Social Worker (LSW) as determined by the Maine State Board of Social Worker Licensure.
  • At least 1 year of experience working with adults with Brain Injuries
  • At least 1 year of case management experience.
  • Must have or be eligible for conditional or full licensure at the Licensed Social Worker (LSW) level as issued by the Maine State Board of Social Worker Licensure.
  • Possession of a valid Class C Maine Motor Vehicle Operator's License.

Responsibilities
