About The Position

The Department of Health and Human Services (DHHS) is seeking a DD Resource Coordinator (Brain Injury Care Monitor) for a full-time position located in Augusta or Portland, with partial telework options. This role is responsible for ensuring appropriate services are provided to adults with Brain Injuries, approving individual service plans, and assuring members of the Sections 18 waiver program meet the requisite level of care. The position involves reviewing and acting upon prior authorization requests, collaborating with service providers, and monitoring service settings for compliance with Home and Community Based Standards. The successful applicant will need knowledge of MaineCare policy, strong critical thinking and customer service skills, and excellent communication abilities to work with diverse stakeholder groups.

Requirements

  • A Master’s Degree in Social Work, Psychology, Special Education, or a related field and one (1) year experience in developmental disabilities or a directly related human services field, OR a Bachelor's Degree Social Work, Psychology, Special Education, or a related field and at least four (4) years’ experience in developmental disabilities or a directly related human services field.
  • Demonstrate a high level of critical thinking and customer service skills.
  • Exhibit excellent communication skills and be comfortable working with varied stakeholder groups.
  • Knowledge of applicable MaineCare policy.

Responsibilities

  • Ensure that appropriate services are provided to adults with Brain Injuries and approve individual service plans
  • Assure members for the Sections 18 waiver program meet the requisite level of care
  • Review and act upon prior authorization requests
  • Collaborate with service providers to assist with overcoming identified problems
  • Review/analyze documents such as care plans, case plans, behavior plans; discuss and document progress and difficulties with the individual and their team
  • Monitors service settings to ensure compliance with HCBS (Home and Community Based Standards)
