About The Position

Northeast Counseling Services is searching for a caring, compassionate person who desires a rewarding and meaningful employment life helping people with mental health issues take charge of their lives and become productive citizens. Case management work is done in communities settings, not in offices. The Blended Case Manager works with the serious and persistently mentally ill population, using a strength-based perspective, to assist clients to obtain necessary services and support. The primary goal is to empower the client to identify their needs and choose the resources to fulfill them.

Requirements

  • Bachelor's degree with major course work in human services or other related social sciences.
  • Must maintain a valid PA Driver's license.
  • Strong communication skills – Ability to clearly communicate with clients, families, and service providers.
  • Active listening – Understanding clients' needs, concerns, and goals.
  • Empathy and compassion – Supporting individuals experiencing serious and persistent mental illness.
  • Resource coordination – Connecting clients with community services such as housing, healthcare, and employment support.
  • Client advocacy – Helping clients access services and navigate systems.
  • Strength-based approach – Identifying and building on clients' abilities rather than focusing only on challenges.
  • Time management – Managing multiple clients and appointments in community settings.
  • Documentation and record-keeping – Maintaining accurate case notes and service plans.
  • Problem-solving – Addressing barriers that prevent clients from accessing needed services.
  • Community outreach – Working with clients in various community environments rather than an office setting.
  • Independence and self-direction – Managing a mobile schedule and working autonomously.
  • Adaptability – Responding to changing client needs and situations.
  • Cultural sensitivity – Respectfully working with diverse populations.
  • Ethical decision-making – Maintaining confidentiality and professional boundaries.
