The Blended Case Manager provides case management services to adolescents, children, and adults who suffer from a serious mental illness or emotional disturbance. An assigned blended case manager works with the client and families to access, link, coordinate and monitor needed services as well as to provide support and advocacy to assist clients in maintaining in their home or community. This role involves completing all reports and documentation in a timely fashion, maintaining quality standards, and participating in an on-call rotation to ensure 24-hour coverage. The position requires effective time management, strong organizational and communication skills, and computer competency. Blended Case Managers are expected to be available to meet with consumers during normal business hours, evenings, and weekends, and an on-call BCM will be available via hotline 24/7. Mileage reimbursement is provided at the state-set rate.
