About The Position

Northeast Counseling Services (NCS) is seeking a compassionate, motivated, and dedicated individual to join our team as a Blended Case Manager. If you are passionate about helping individuals with mental illness achieve their goals, increase independence, and improve their quality of life, we encourage you to apply. This position provides services in community settings rather than an office environment, allowing you to directly support individuals where they live, work, and participate in their communities. The Blended Case Manager works with individuals experiencing serious mental illness using a strengths-based, recovery-oriented approach. The primary goal of this position is to empower clients to identify their needs, access resources, and achieve greater independence through coordinated support and advocacy.

Requirements

• Valid PA Driver's license
• High School Diploma or equivalent
• Minimum of 12 college credit hours in Sociology, Human Services, Psychology, or another social science field
• Two (2) years of experience in Human Services
• One (1) year of experience providing direct client contact
• Successful completion of an FBI Clearance, PA Criminal Clearance and Child Abuse Clearance

• Bachelor's Degree in Social Work, Psychology, Human Services, Sociology, or a related field
