About The Position

Professional Services Group is hiring a bilingual English/Spanish Service Coordinator for the Children's Long-Term Support (CLTS) Waiver Program in Waukesha County. The role provides case management and service coordination to children and families seeking counseling and referrals related to intellectual or developmental, physical, or mental health disabilities. Coordinators work with clients and families to assess needs, develop casework plans, and coordinate social, community, and other services. The position is based at the Waukesha County Human Services building and requires local travel for client home visits; mileage is reimbursed. Hybrid or work-from-home work may be available after six months. The schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m., with occasional evenings to accommodate families.

Requirements

Bilingual English/Spanish proficiency.
Bachelor's degree in sociology, social work, psychology, or a closely related field.
At least one year of professional social work experience.
Reliable transportation for daily use, a valid driver's license, and automobile insurance.

Responsibilities

Receive new client referrals and make initial contact.
Assess client needs through interviews, collateral contacts, and other investigations.
Develop casework plans with clients and perform casework services.
Maintain a caseload of clients with personal, family, or socioeconomic problems, attending to related protection and safety needs.
Provide counseling, intervention, and case management by coordinating social, community, and other services.
Conduct in-person home visits to assess needs and maintain client contact.
