About The Position

Wider Circle is seeking a Bilingual Telehealth Physician for their Pennsylvania pilot program, Wider Care+. This program focuses on delivering community-rooted, relationship-driven care for older adults and clinically complex populations. The physician will serve as the clinical entry point, conducting comprehensive telehealth assessments to set the foundation for ongoing intervention by a coordinated team of physicians, Community Health Workers, Licensed Clinical Social Workers, and care navigators. This role is designed for clinicians who are interested in innovative care delivery, population health, and collaborative, whole-person models of care, moving away from traditional practice pressures.

Requirements

• MD or DO with active, unrestricted Pennsylvania license
• Substantial experience with Medicare/Medicaid populations, or complex chronic disease management
• Skilled at telehealth assessment and building rapport virtually
• Strong clinical judgment, comfort with complexity and ambiguity, and interest in care coordination
• Comfortable working in an iterative, startup-like pilot environment

Nice To Haves

• Board-certified or board-eligible in Family Medicine, Internal Medicine, Geriatrics, or related primary care specialty preferred

Responsibilities

• Conduct comprehensive telehealth initial visits for enrolled members (15-20 min)
• Synthesize complex medical and social histories and identify clinical priorities and risks
• Create concise, actionable clinical plans and handoffs for Community Health Workers, LCSWs, and care navigators
• Document findings and recommendations clearly in the EMR to guide ongoing care
• Provide clinical guidance on escalation pathways and program workflows during the pilot
• Build rapid clinical rapport and trust with members and caregivers
