About The Position

NADAP is seeking a Bilingual Spanish Care Manager II to join their Health Homes Care Management team. This role will be based out of Patchogue, NY, with fieldwork across Suffolk County. The Care Manager will assist individuals with medical and behavioral health conditions by providing field-based support. This includes services such as assessment, care planning, connections to primary and specialty care providers, service coordination with treatment providers, client education on diagnoses, medications, and treatments, and linkages to community-based resources and entitlements. NADAP is a multiservice non-profit agency dedicated to helping people with medical, behavioral health, and social service needs become self-sufficient, serving disadvantaged populations in New York City and Long Island.

Requirements

Bachelor’s Degree in human services, social work, or related field OR Associate’s degree with 1 year of case management experience OR High School Diploma with 2 years of case management experience
Excellent organizational, communication, and computer skills
Personal vehicle and valid driver's license required
Bilingual Spanish proficiency
