About The Position

NADAP is seeking a Care Manager II to join their Health Homes Care Management team. This position will be based out of Patchogue NY with field work across Suffolk County. Care Managers help individuals achieve improved healthcare outcomes by providing field-based support to individuals with medical and behavioral health conditions. NADAP, Inc. is a multiservice non-profit agency dedicated to helping people with medical, behavioral health, and social service needs to become self-sufficient. NADAP programs assist disadvantaged populations in New York City and Long Island. NADAP's services include health insurance enrollment, assessment, care coordination, case management, professional training, job preparation, and placement services. Requirements

Bachelor’s Degree in human services, social work, or related field OR Associate’s degree with 1 year of case management experience OR High School Diploma with 2 years of case management experience
Excellent organizational, communication, and computer skills
Personal vehicle and valid drivers license required
