About The Position

The University of Colorado, College of Nursing is seeking a Bilingual Primary Care Referral Coordinator for a full-time (1.0 FTE) position within the Office of Clinical and Community Affairs at the Sheridan Health Services clinics located in the Fort Logan neighborhood of Denver. The Bilingual Primary Care Referral Coordinator performs both referral and administrative duties to support comprehensive and high-quality patient care. This position will represent Sheridan Health Services professionally and positively to enhance and promote Sheridan’s mission to provide quality patient-centered care. Sheridan Health Services is a Patient-Centered Medical Home, where the patient is at the center, incorporating the values and beliefs of the patient in all decision-making. Sheridan Health Services is a multi-site, nurse-managed federally qualified community health center, funded through the Health Resources and Services Administration Bureau of Primary Healthcare, and is a collaborative effort between the College of Nursing and the Sheridan School District. The Family Health Clinic is located in southwest Denver on the Fort Logan campus and provides services to adults and children, 65%25 of whom identify as Latinx, and 42%25 of whom are best served in another language, with Spanish being the predominant second language. Services offered include primary care across the lifespan; prenatal and postpartum, gynecology, dentistry, pharmacy, behavioral health, and outpatient substance use treatment. Sheridan Health Services is a Patient-Centered Medical Home, and as such, we uphold a model of care that places the patient at the center, incorporating the values and beliefs of the patient in all decision-making. We extend the reach of the clinic through public health outreach, where nursing students provide community outreach, health education, and home visitation services.

Requirements

  • Three (3) years of general clerical experience. One (1) year of this experience must have included care coordination in a clinical setting and/or an insurance company health plan referral unit.
  • A combination of related education and/or relevant general clerical experience equal to three (3) years.
  • College or university coursework appropriate to the work assignment may be substituted on a year-for-year basis.
  • Applicants must be legally authorized to work in the United States without requiring sponsorship.
  • Must be willing and able to obtain current BLS/CPR certification by date of hire.
  • Must be willing and able to speak Spanish and English on a daily basis to communicate clinical terminology.

Nice To Haves

Responsibilities

  • Process routine, urgent, and emergent referrals for patients in the Adult Primary Care Service Line.
  • Verify patient insurance eligibility, communicating with nurse practitioners and the health care team as needed.
  • Obtain prior authorization or pre-certification when needed.
  • Assist with scheduling appointments consistent with patients' insurance plans.
  • Prepare patient paperwork for the provider to review and finalize.
  • Track referrals until completion per clinic policy.
  • Connect with specialty care clinics, community resources, and services.
  • Communicate appropriate diagnostic and clinical information as needed to complete referrals thoroughly.
  • Uphold HIPAA policies and procedures to protect patient health information.
  • Attend community events to assist in care coordination or perform community outreach.
  • May serve as the point of contact for community partners to manage incoming referrals and schedule new or existing patients with more than one service line.
  • Monitor incoming faxes daily and upload progress notes, scheduling notices, imaging results, and specialty referral communication to the EMR.
  • Assist in managing the transitions of care from the Emergency Department and hospitalizations to primary care.
  • Assist in scheduling clinic visits as needed.
  • May be asked to prepare or manage Medical Records Requests.
