About The Position

The Family Resource Case Worker (FRCW) provides essential case management support to families within the MST team. This role involves assessing family needs and collaborating with community programs to ensure comprehensive support and improved family stability.

Requirements

  • Bachelor's degree in Social Work, Psychology, or a related field.
  • Experience in case management or social services, particularly with families and children OR Strong knowledge of community resources and ability to effectively collaborate with schools, medical providers, and other community programs.
  • Excellent writing, communication, interpersonal, problem-solving, time management and organizational skills.
  • Ability to work independently and as part of a team.
  • Proven self-management abilities including meeting deadlines, prioritizing multiple tasks efficiently, timely completion of documentation and maintaining accurate and up-to-date case records.
  • Ability to conduct home visits and perform safety assessments.
  • Ability to work effectively with a diverse, multidisciplinary team as well as a diverse client population.
  • Ability to receive feedback to professionally grow and/or improve and be flexible with programmatic needs and changes.

Responsibilities

  • Conduct intake assessments and gather consent forms for new cases, ensuring timely case openings and completeness of information.
  • Provide clear explanations of MST therapy and its benefits to families to achieve high levels of engagement.
  • Partner with therapists to develop and implement interventions to overcome barriers families face with session pacing and work collaboratively towards treatment goals.
  • Become an expert on community resources and assist families in accessing services such as prosocial activities, public benefits, housing, legal, education, and medical services.
  • Collaborate with community resources to support family needs.
  • Create, manage, and process cases in Evolv, Promis, Connections, and Ebase, ensuring all contacts are documented, up-to-date, and accurate.
  • Request and obtain school, medical, dental, or other applicable health records on an ongoing basis for children under 18 or relevant family members.
  • Provide consistent support and advocacy for families through monthly or more frequent in-person visits, and attend meetings, appointments, and conferences to provide support and address barriers.
