About The Position

The Nuestro Case Manager supports the Highwood community by connecting individuals to resources, benefits, and essential services. This role collaborates with expressive therapists to assist with programming, including facilitating after-school groups and supporting senior programming, as well as engaging in community outreach. The Nuestro Case Managers advocate for clients’ health and well-being by coordinating care with healthcare professionals and developing appropriate treatment plans. They also ensure continuity of care by creating transition plans for clients following discharge from hospitals or mental health centers. Additional responsibilities include carrying out duties assigned by the program Director.

Requirements

  • Minimum requirement of a Bachelor’s degree in Social work, Psychology, or another human service-related field
  • Three professional references that reflect ability to perform clinical work
  • 1 Year minimum of working in Mental Health experience required
  • Demonstrated awareness of multiple cultures and cultural impact on the delivery of service is a must.
  • Demonstrate competence in interdisciplinary and inter-agency collaboration
  • Demonstrated administrative, supervisory, and organizational skills
  • Advanced oral and written communication skills
  • Ability to think critically and analytically and to problem solve in both short and long term contexts
  • Bachelor's degree in human service field
  • At least 1 year relevant experience working with individuals experiencing mental illness

Responsibilities

  • Provide face-to-face annual mental health assessments as needed, ensuring all required documentation meets state health standards
  • Support coordination with community organizations as needed
  • Facilitate client referrals for services aligned with assessed needs, physician recommendations, and client goals
  • Provide brief crisis intervention, stabilization services, and supportive counseling
  • Collaborate regularly with expressive therapists and fellow case managers
  • Provide backup coverage for other case managers and/or expressive therapists
  • Carry out additional duties as assigned by the Program Director
  • Demonstrate leadership initiative or assume leadership responsibilities when requested
  • Participate in community events and outreach efforts
  • Attend and engage in supervision with the Program Director
  • Participate in staff development and training seminars
  • Submit timely responses to Program Director requests for data or special projects
  • Coordinate vacation and time-off requests with the Supervisor
  • Complete all clinical documentation and reports accurately and within required timelines, including: Treatment plans (every 6 months), Service tickets and progress notes (within 3 days)
  • Complete referral, transfer, and discharge documentation in a timely manner: Referral and transfer documentation completed immediately upon need, Discharge summaries and instructions completed within three working days
  • Complete and update annual mental health assessments and related documentation in Smart Care
  • Ensure accuracy in all treatment plans, referrals, transfers, and discharge paperwork
  • Participate in agency orientations and training programs
  • Maintain client confidentiality and adhere to agency ethical standards
  • Meet productivity goals as established by the supervisor
