About The Position

By The Bay Health, a non-profit established in 1975, set the standard for hospice in the U.S. by emphasizing the role of the patient in making important medical decisions. Their spectrum of home-based services now includes Skilled Home Health Care, Palliative Care, Adult Hospice Care and Pediatric Care. The team approach strives to address practical, social, emotional and spiritual aspects of care, with the goal to maximize quality of life for patients, caregivers and families. The S.F. Team has a diverse and dedicated team of clinicians, many of whom are multilingual in English, Mandarin, Cantonese and Taishanese. By The Bay Health serves a wide population of monolingual Chinese speaking patients and their families, providing comfort and support in their own language and cultural milieu. They aim to employ clinicians with the passion and language/cultural skillsets to continue providing the highest level of care possible as they grow their census of culturally diverse patients. They are currently seeking a Registered Nurse to serve as a Hospice Nurse Case Manager, responsible for the provision of skilled nursing assessment, planning and care to maximize the comfort and health of patients and families.

Requirements

  • Bi-lingual in Cantonese, Mandarin, or Taishanese
  • One year experience as a professional nurse in the past three years OR Bachelor’s degree in Nursing from an accredited School of Nursing
  • Current license as a registered professional nurse in the State of California
  • Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car

Nice To Haves

  • Experience in acute care/ home health and/or hospice setting
  • Public Health Nursing certification
  • HPNA certification

Responsibilities

  • Provide care in the home and work as a member of the interdisciplinary team comprised of nurses, social workers, chaplains, hospice aides and physicians
  • Coordinate all patient/family services and prioritization of needs, including implementation of the individualized plan of care
