They offer a spectrum of home-based services including Skilled Home Health Care, Palliative Care, Adult Hospice Care, and Pediatric Care. Their team approach focuses on addressing practical, social, emotional, and spiritual aspects of care to maximize the quality of life for patients, caregivers, and families. The San Francisco team is diverse and multilingual, serving a significant population of monolingual Chinese-speaking patients and their families by providing culturally sensitive care in their own language. The organization seeks clinicians with passion and language/cultural skillsets to continue this high level of care. They are currently seeking a Registered Nurse to serve as a Hospice Nurse Case Manager, responsible for skilled nursing assessment, planning, and care to maximize patient and family comfort and health.

Requirements

  • One year experience as a professional nurse in the past three years or Bachelor’s degree in Nursing from an accredited School of Nursing.
  • Current license as a registered professional nurse in the State of California.
  • Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car.
  • Bi-lingual in Cantonese, Mandarin, or Taishanese required!

Nice To Haves

  • Experience in acute care/ home health and/or hospice setting preferred but not required.
  • Public Health Nursing certification highly desirable.
  • HPNA certification preferred.

Responsibilities

  • Provide care in the home and work as a member of the interdisciplinary team comprised of nurses, social workers, chaplains, hospice aides and physicians.
  • Coordinate all patient/family services and prioritization of needs, including implementation of the individualized plan of care.
