Our spectrum of home-based services now includes Skilled Home Health Care, Palliative Care, Adult Hospice Care and Pediatric Care. Our team approach strives to address practical, social, emotional and spiritual aspects of care, with the goal to maximize quality of life for our patients, caregivers and families. Our S.F. Team has a diverse and dedicated team of clinicians, many of whom are multilingual in English, Mandarin, Cantonese and Taishanese. We are proud to serve a wide population of monolingual Chinese speaking patients and their families with the ability to meet them where they are, providing comfort and support in their own language and cultural milieu. We strive to employ clinicians who have the passion and language/cultural skillsets to continue providing the highest level of care possible as we grow our census of culturally diverse patients. We are currently seeking a Registered Nurse to serve as a Hospice Team Nurse. This person will be responsible for the provision of skilled nursing assessment, planning and care to maximize the comfort and health of patients and families. Schedule: Monday- Friday; 8:30am - 5:00pm Location: San Francisco & Marin Counties Bi-lingual in Cantonese, Mandarin, or Taishanese required!
Requirements
- One year experience as a professional nurse in the past three years or; Bachelor’s degree in Nursing from an accredited School of Nursing
- Current license as a registered professional nurse in the State of California
- Bi-lingual in Cantonese, Mandarin, or Taishanese required!
Nice To Haves
- Experience in home health and/or hospice setting preferred
- Public Health Nursing certification highly desirable
- HPNA certification preferred
Responsibilities
- Assume primary responsibility for a patient/family using the nursing process to assess, plan, implement and evaluate patient/family needs, goals, and interventions
- Coordinate all patient/family services and prioritization of needs with the members of the transdisciplinary team
- Use a case management approach and refer to other services as needed
- Coordinate the implementation of the plan of care and insure care coordination and case management for each patient
