About The Position

Kline Galland Hospice is part of a non-profit organization with an over 110 year history in providing outstanding care for the King County community. We are looking for an experienced, motivated Bereavement Coordinator to join our Hospice team on a Part time basis 3 days a week.

Requirements

  • Master's degree in Social Work, Counseling, Divinity, or related field plus specialized training in grief and loss, and bereavement counseling.
  • 2 years experience working in end of life and/or bereavement care.
  • Counseling experience, both individual and group.
  • Working knowledge of Community resources including groups and individual counseling options.
  • Strong verbal and written communication skills.
  • Strong organizational skills with the ability to work independently or as part of the interdisciplinary team.
  • Computer skills for reports and documentation.
  • Valid driver's license, auto insurance, dependable vehicle.

Responsibilities

  • Coordinates assessment and delivery of grief counseling needs and services to family and caregivers of hospice patients.
  • Assessment of family/caregiver for potential emotional risk
  • Plans for Bereavement Program development and expansion through ongoing evaluation of Program.
  • Provides bereavement services to hospice staff through one-on-one support and staff development meetings and activities.
  • Provides linkage and referral for families/caregivers to outside counseling resources as indicated.
  • Provide grief and loss services in response to Community needs as requested.
  • Participate in hospice team activities such as Interdisciplinary Team meetings, annual Memorial Service planning, QAPI projects, and others as indicated.
  • Creating and following appropriate Bereavement Care plans.
  • Ensure that the Medicare required elements of the Bereavement program are accurately completed in a timely manner.
  • Documents the activities for the bereavement program to include documentation into the patient record
