About The Position

Good Samaritan Hospice, a ministry of Concordia Lutheran Ministries, is seeking a compassionate Bereavement Coordinator to support families and loved ones before and after the loss of a loved one. The role provides emotional support, grief education, counseling, and ongoing bereavement services to help people navigate the grieving process. This is a part-time position working 24 hours between Monday and Friday, with occasional weekend events. The position is based in the Valencia Office.

Requirements

Strong interpersonal, communication, and active listening skills.
Ability to demonstrate compassion, empathy, and professionalism when working with individuals experiencing grief and loss.
Ability to work independently while collaborating effectively with an interdisciplinary team.
Valid driver's license and reliable transportation as needed for job responsibilities.

Nice To Haves

Bachelor's degree in Social Work, Counseling, Psychology, Human Services, or a related field preferred.
Previous experience in hospice, grief counseling, bereavement services, or a related field preferred.

Responsibilities

Provide bereavement support, counseling, and follow-up services to families and loved ones of hospice patients.
Assess bereavement needs and develop individualized plans of care.
Maintain regular contact with bereaved family members through phone calls, correspondence, and visits as appropriate.
Facilitate grief support groups and educational programs.
Provide resources, education, and referrals to community support services when needed.
Maintain accurate and timely documentation in accordance with hospice policies and regulatory requirements.
Collaborate with the interdisciplinary hospice team to ensure comprehensive support for patients and families.
Participate in hospice memorial services and other bereavement-related activities.
