About The Position

Integrated Care Solutions NRIO, a division of Bayshore HealthCare, offers community-minded, progressive brain injury rehabilitation focused on empowering individuals to enhance their quality of life. They utilize a person-and family-centered care (PFCC) approach, providing respectful, compassionate, and culturally safe care. This role involves supporting clients with behavioral and psychological symptoms resulting from cognitive impairment, dementia, and/or acquired brain injury (ABI) under the direction of a Behaviour Therapist and in collaboration with a Clinical Director/Psychologist.

Requirements

  • Post-secondary education in behaviour sciences, mental health/addictions, psychology, and/or related areas.
  • Standard First-Aid + CPR-C.
  • Minimum G2 driver’s license.
  • Minimum 2 years assisting with behaviour assessments and collaborating on behaviour interventions.
  • Experience working with individuals with various types of dementia, delirium, cognitive issues, and complex behavioural challenges.
  • Knowledge of psychogeriatric assessment tools and behavioural, cognitive, psychosocial, and environmental assessments relating to responsive behaviours.
  • Practical experience supporting individuals in an acute care and/or mental health care settings; application of appropriate crisis, de-escalation, and behavioural management strategies.
  • Ability to work independently and in collaboration with others.
  • Excellent clinical-reasoning, critical thinking, and problem-solving skills.
  • Excellent interpersonal and conflict management skills.
  • Creativity, flexibility, positive attitude.
  • Ability to work well under pressure.
  • Comfort with addressing issues such as verbal aggression, physical and environmental aggression, elopement, socially and sexually inappropriate behaviours.
  • Strong communication skills (oral and written).
  • Technological proficiency: use of MS Office (including Word, Excel and PowerPoint, Outlook, Teams).

Nice To Haves

  • Demonstrated experience working with dual diagnosis is considered an asset.
  • Personal Support Worker (PSW) experience is considered an asset.
  • Transitional care support experience is considered an asset.

Responsibilities

  • Provide transitional care support (e.g., from hospital to a transitional care unit, supportive living residence, or home).
  • Model appropriate behavior, provide positive reinforcement and corrective feedback.
  • Assist with Activities of Daily Living (ADLs), including bathing, transfers, toileting, physical assistance, and participation in activities. Light housekeeping as needed.
  • Assist with connection to community supports for specific populations (e.g., diabetes care, recreational programs, addictions support, housing and food insecurity).
  • Assist with and implement behavioral and rehabilitation interventions and plans as prescribed by the multidisciplinary team.
  • Collect data on client behavior through direct observation, interviews, and standardized assessments.
  • Assist with administration of medication programs.
  • Complete documentation/reports following company and provincial guidelines.
  • Report significant clinical findings to the Clinical Director.
  • Participate in team meetings.
  • Provide educational support to staff and caregivers regarding behavioral care plans.
  • Participate in team and/or organization-wide committees.
  • Participate in the evaluation of risks associated with maladaptive behaviors and contribute to mitigation plans.
  • Maintain confidentiality of client and corporate information.
  • Ensure the protection of client rights and carry-out an advocacy role.
  • Remain current on clinical best-practices and evidence-based interventions.
  • Participate in ongoing internal and/or external continuing education activities.
  • Adhere to Bayshore Policies and Procedures.
  • Participate in quality activities and continuous improvement initiatives.
  • Participate in proactive Health & Safety activities and report any risks or concerns.
  • Complete other tasks as requested.
