About The Position

This role is responsible for providing ongoing assessment, crisis intervention, individual, family, and group therapy, as well as case management for clients with behavioral health disorders and/or disabilities. The therapist will develop and monitor service plans, track treatment progress, document interactions, and maintain clinical records and other reporting data. This position also involves coordinating with internal and external supportive services and collaborating with a treatment team. As a member of the Homeless Services Team, the therapist will be embedded at the Murphy Center, offering screening, assessments, resource connection, care collaboration, crisis intervention, intakes, case management, mental health services, and substance use disorder services to individuals experiencing homelessness, irrespective of their ability to pay.

Requirements

  • Master's degree in social work, counseling, psychology, or related field from an accredited college or university
  • Clinical licensure (LCSW, LPC, LMFT, CAS Track) or clinical licensure candidacy
  • Valid Colorado Driver's license or ability to obtain within 30 days of hire and a clean driving record
  • At least one year of experience in the behavioral health field

Responsibilities

  • Provide services to clients as assigned by Administrative/Clinical Supervisor.
  • Provide/facilitate a wide variety of clinical services including individual, family and group activities to achieve treatment plan goals.
  • Conduct crisis intervention via telephone, face-to-face.
  • Intervene and de-escalate crisis situations with clients.
  • Assess the risk of harm to self or others and arrange needed resources.
  • Assess behavioral health needs of clients and families; develop treatment plans and goals.
  • Perform case management services as needed to assist clients to meet their goals.
  • Refer clients to supportive services.
  • Document treatment plans, progress reports, group notes, individual notes, case management notes, letters, faxes and phone calls in Electronic Health Record.
  • Follow the documentation policy and procedures.
  • Collaborate with partner agencies to support clients in carrying out treatment/service plans.
  • Follow agency productivity standards as outlined by SummitStone Health Partners.
  • Perform other duties as assigned.
