THIS POSITION WILL SUPPORT BOTH DOWNTOWN LOS ANGELES (90057) & COMPTON (90221) LOCATIONS - MONDAY - FRIDAY 8AM - 5PM This is an opportunity for a mission-driven team player to provide psychiatric and behavioral health support to participants and guidance on behavioral health to staff members. In this role, you will play a key role within the Interdisciplinary Team and will work collaboratively with team members to assess, plan, provide primary care, and educate our interdisciplinary team on behavioral health interventions into plans of care. You will partner with internal team members, community-based behavioral health specialists, provider specialists and various facilities and community-based organizations. This is a center-based role that reports to the Medical Director.

Requirements

  • Graduate of an accredited Psychiatric Mental Health Nurse Practitioner program.
  • Active Psychiatric Mental Health Nurse Practitioner State License without restrictions.
  • State CDS Certification and DEA License.
  • NPI license.
  • 3+ years working with behavioral health in an elderly or frail and complex population.
  • CPR and First Aid certification (within 60 days of employment).
  • Proof of a valid CA driver’s license, personal transportation, good driving record and auto insurance as required by State law.

  • Passion and mission orientation for serving high-risk seniors and frail older adults.
  • Strong clinical assessment, diagnostic, counseling, and treatment skills across a range of psychiatric and behavioral health conditions.
  • Ability to collaborate effectively within an Interdisciplinary Team and with internal and community-based behavioral health specialists.
  • Ability to educate and train staff on behavioral health assessments, goal setting, and interventions.

Responsibilities

  • Provide psychiatric consultation, psychiatric illness evaluation, counseling, and treatment for participants in collaboration with PACE Physicians and Community-Based PACE Physicians; makes referrals to Psychiatrists as appropriate.
  • Perform in-person and/or telehealth comprehensive psychiatric assessments on participants and collaborate with the Interdisciplinary Team (IDT) to develop a comprehensive care plan that includes goal concordant care for each participant.
  • Partner with the Mental Health Counselor to support IDT meetings and the care planning process to identify knowledge gaps around behavioral health issues and educate IDT members on behavioral health assessments, goal setting, and interventions, including mental disorders, substance abuse and behavioral manifestations of dementia.
  • Work with clinicians and staff to approve integration of behavioral health interventions into plans of care.
  • Establish relationships with and ensure 2-way communication with community behavioral health specialists.
  • Coordinate schedules and work with consulting Geriatric Psychiatrists to ensure appropriate exchange of information and utilization of resources.
  • Follow participants admitted to inpatient psychiatric facilities to ensure identified issues are addressed and that follow-up behavioral health interventions are appropriate.
  • Evaluate participants whose behavioral health diagnoses or issues are not clear to establish a diagnosis and/or plan of care.
  • Participate in Team Member training on behavioral health.
