The Behavioral Health Patient Care Companion ensures a secure and safe environment by maintaining continuous awareness of safety hazards and patient observation. This role involves never leaving an assigned patient unattended and ensuring continuous monitoring before leaving the room for breaks or shift changes, or to accompany a patient on transport. The companion follows established policies and procedures for various precautions (suicide, seizure, falls, elopement, restraining) and provides restraint alternatives when appropriate, such as diversional activities. They also adhere to emergency plans and codes, and accompany assigned patients at all times during hallway walks, bathroom visits, or showers. Additionally, the companion provides general patient care duties as directed by the registered nurse, including assisting with personal care, ambulation, positioning, toileting, and maintaining patient supplies. When assigned to general floor duty, they respond to call lights and communicate patient requests to the RN. The role also involves meeting nutritional needs by reviewing dietary orders, assisting with meals and feeding, providing tray setup, assisting with menu selection, and communicating intake/output information to the RN. Patient confidentiality and respectful communication with patients, families, and health professionals are paramount. The companion supports patient/family teaching by collaborating with the healthcare team and seeking RN guidance for clinical information or family questions. They contribute to the patient care plan by documenting patient information, communicating needs and observed changes to the RN, and providing verbal shift reports. Acting as a patient/family advocate, they offer comfort, assistance, and information, and help orient patients and families to the environment. The companion demonstrates awareness of the patient’s plan of care, including activity, diet, and social needs. They also perform tasks to maintain patient care standards and efficient unit operations, respond to emergencies, maintain technical skills through inservices, and practice infection control procedures.
