POSITION SUMMARY: The Behavioral Health Nursing Supervisor will facilitate and ensure a structured, thoughtful, planned and integrated plan of care and process for all Community Health, Behavioral Health patients. They will oversee the clinical staff as well as BH PAC staff within our behavioral health. They will oversee patient care, staff development, compliance, and quality improvement, while ensuring that clinical standards and regulatory requirements are met. They will help in the identification of patients at risk for opioid abuse or addiction. Create access into our MAT program as an integrated component of effectively managing the patient’s overall health. They will also oversee all medical operations within the Behavioral Health Department and under work with the Director of Behavioral Health & MAT Services in ensuring best practices are implemented.

Requirements

• Current Vermont RN License (may consider LPN)
• CPR Certification.
• Prior experience working in a nursing position required
• Prior case management experience in a similar outpatient setting preferred.
• Experience in using a variety of electronic medical record and ability to learn other systems, basic keyboarding skills and email communication.
• Managed-care experience a plus

Responsibilities

• Facilitate and ensure a structured, thoughtful, planned and integrated plan of care and process for all Community Health, Behavioral Health patients.
• Oversee the clinical staff as well as BH PAC staff within our behavioral health.
• Oversee patient care, staff development, compliance, and quality improvement, while ensuring that clinical standards and regulatory requirements are met.
• Help in the identification of patients at risk for opioid abuse or addiction.
• Create access into our MAT program as an integrated component of effectively managing the patient’s overall health.
• Oversee all medical operations within the Behavioral Health Department and work with the Director of Behavioral Health & MAT Services in ensuring best practices are implemented.
• Training/Education of new medical/ clerical BH staff
• Coordinates MAT Care Management services at all Community Health sites that provide MAT services
• Reviews clinical documentation for accuracy and continuity with quarterly chart audits
• Manages Community Health MAT patient and provider list to ensure adequate testing per VPMS guidelines
• Facilitates timely and accurate urine drug screen follow-up and guidelines for controlled substances.
• Oversees the color line and UDS management and coordination of lab services
• Attend MAT State wide meetings and provides any pertinent documentation for any training/education
• Reporting to agencies for harm reduction supplies (harm reduction bags, Narcan, lock bags/ boxes)
• Inventory and inspection of the rapid response bag, AED, Code Cart and all other medical items within the building.
• Conducts performance evaluation at regular intervals
• Creates/ adjusts EMR workflows, manages inventory/ ordering of medical supplies
• Conducts interviews for new incoming clinical and clerical staff
• Runs monthly team meetings, conduct monthly/ biweekly supervision with team members and acts as a mentor for staff.
• Supports the Director of Behavioral Health & MAT Services in any other support as needed.
• Provide direct nursing and care management services to BH patients
• Act as liaison between MAT and Medical provider to ensure that patient is up to date on medical needs.
• Provides support and coordination to our patients in preventing barriers to care and promotion of community resources and healthcare services.
• Provide follow-up phone calls and any other services post discharge to BH/MAT patients that are discharging from an inpatient or residential program.
• Coordinates Behavioral Health chronic care management services to eligible MAT patients.
• Work collaboratively with patients in psychiatric and substance abuse programs.
• Work with HUB and SPOKE programs, SASH, VCCI, RMH and other community resources to coordinate care
• Cover BH refills/ CM roles as needed due to absences and vacations or vacancies
• Participate in team meetings for high utilizers
• Provide clinical support to BH staff and RQI training
• Collaborate with Director of Clinical Services, Clinical educator, and Clinical support specialist to implement new workflows and support skills within the team
• Keeping up to date on all medical equipment and ensuring that it remains in working order.
• Oversight of exam rooms set up/ equipment installation.
• Cross training staff for multiple roles
• Other duties as assigned
