About The Position

This role involves providing direct patient care within a behavioral health setting, focusing on assessments, care plan development, and medication administration. The nurse will manage a caseload, coordinate with the clinical team, monitor patient progress, and educate patients and caregivers. Key responsibilities include ensuring continuity of care through effective communication with physicians and care teams, supervising HHAs and LPNs, and maintaining accurate documentation in compliance with regulations like HIPAA.

Requirements

• Active RN license
• Graduate of an accredited nursing program (BSN preferred)
• Home health or related behavioral health clinical experience
• Strong communication, organization, and critical thinking skills
• Current CPR certification
• Valid driver’s license and reliable transportation

Responsibilities

• Provide direct patient care, including assessments and care plan development
• Manage a caseload and coordinate care across the clinical team
• Administer medications and treatments per physician orders
• Monitor patient progress and adjust care plans as needed
• Educate patients and caregivers on care and self-management
• Communicate with physicians and care teams to ensure continuity of care
• Supervise and support HHAs and LPNs
• Complete accurate and timely documentation (OASIS, visit notes, orders)
• Ensure compliance with policies, regulations, and HIPAA standards
