About The Position

This is a part-time position for a Behavioral Health Nurse working afternoon/evening shifts. The role involves providing direct patient care within a home health or related behavioral health setting. Responsibilities include patient assessment, care plan development, medication administration, patient education, and interdisciplinary communication. The nurse will also supervise HHAs and LPNs and ensure accurate documentation and compliance with regulations.

Requirements

• Active RN license
• Graduate of an accredited nursing program (BSN preferred)
• Home health or related behavioral health clinical experience
• Strong communication, organization, and critical thinking skills
• Current CPR certification
• Valid driver’s license and reliable transportation

Responsibilities

• Provide direct patient care, including assessments and care plan development
• Manage a caseload and coordinate care across the clinical team
• Administer medications and treatments per physician orders
• Monitor patient progress and adjust care plans as needed
• Educate patients and caregivers on care and self-management
• Communicate with physicians and care teams to ensure continuity of care
• Supervise and support HHAs and LPNs
• Complete accurate and timely documentation (OASIS, visit notes, orders)
• Ensure compliance with policies, regulations, and HIPAA standards
