The Behavioral Health Integration Coordinator (Adult) will manage a dynamic case load and serve as a hub for the collaborative delivery of mental health, primary care as well as specialty care services within the Strong Futures Program. The Behavioral Health Integration Coordinator will be a link for patients transitioning from inpatient acute care, into a continuum of care services that support their on-going recovery goals and objectives. They will ensure patients and families have access to an array of community resources as well a transitional plan within 6 months of their treatment initiation. Within the Behavioral Health Home Program, the coordinator will work closely with patients in the community, ensuring a delivery of care that addresses all aspects of physical and mental health.
